There is no definition that describes “big face” or “small face”. As to what size the face is considered to be “big”, there is also no definite answer. So “big face” and “small face” are relative concepts. A giving face situated in deferent people can be judged as “big” or “small” respectively. The same face on same people but judged by deferent viewers the face could be “too big” or “too small”. From a plastic surgeon’s point of view once the face is harmonic to the rest of the body the size of the face is good. It is not necessary to make the face as small as possible just because a small face may be more attractive. Everything has its range of tolerance. The size of the face is the same. If a face is reduced too much beyond its range of tolerance it may not be normal, not mention beautiful. No body planed to make their face too small to be accepted by others. In real life we do encounter candidates who reduced their face too much by surgery. This is possible because no body, even no surgeon could exactly predict how much reduction for a giving face will make it match a rest of the body in size perfectly.

The beautiful girl on this page believed that her face was too big and then came to Dr. Fushun Ma for surgical help. Upon seeing her Dr. Ma realized that the size of her face is not proportional to her whole body and to the facial organ’s arrangement on her face. A face reduction surgery could improve her feature. So Dr. Ma did Zygoma and Zygomatic Arch Reduction, Masseter Muscle Partial Resectomy and Mandible Bone Sculpture Reduction for her. The size of her face is much smaller after surgery than before surgery. And her facial type also changed from a “diamond square shape” to a “oval shape”. In the front view this beautiful girl’s face type changed obviously. Her before surgery face type is a “diamond square” one. Overall her face is square and the lateral protrusion of the cheek bone made her facial outline in a diamond shape in somehow. If only a mandibular reduction and a masseter muscle resectomy surgeries are performed to narrow the lower part of her face the middle portion of her face would look even wider making her facial outline not natural. So in order to reduce this kind of face naturally cheek bone reduction should be introduce together.

With the first look at the picture one may only notice that her lower face is pointer and narrower due to the mandible reduction and masseter muscle resectomy. However the middle face change caused by the cheek bone reduction can easily be ignored at the first glance. A careful observation of the photos will reveal that the lateral protrusion hump in the middle face disappeared, the exposed areas of both side of the ears are bigger, the facial width lateral to the brows is narrower on both side and the indentations in the temporal and cheek areas are also leveled off. All those changed indicate the result of the zygomatic bone and zygomatic arch reduction surgeries, not just from a modification of the pictures. Dr. Fushun Ma realized that after cheek bone reduction surgery the width of the middle face could be reduced by 3CM, equals the average width of an eye and one fifth of the whole face if using the rule of facial width is five time as the width of the same person’s eye. By this degree of reduction the post surgical result in real person is very obvious. However when comparing the before and after surgery photos the change may still not sharp as it should be in real person. But the phenomenon described above indicate the effect of a successful cheek bone reduction surgery.

In the angled view this beautiful girl’s face is slimmer and less bony in general. The facial outline at the cheek bone level is flatter. The lower facial width in the front view reduced greatly by mandible sculpture reduction and partial masseter muscle resectomy, however in this angle view her mandible angle is still exist and the mandible line is still there in a natural shape, no “horse face” deformity or unevenness is formed by surgeries. Mandible sculpture reduction surgery reduces the size according to its original shape together with the partial masseter muscle resectomy guaranteed this natural change of her face reduction without any traces of man made modification.

In back to front lateral view the outline formed by the left cheek bone is also straighter, plus the change of the right side cheek bone outline, both side of the facial outline at the level of the cheek bone are reduced to a flatter shape. Pictures in this angle of view reveal that the dividing line between the face and the neck is still vivid after surgery and the mandible angle is still exist only the facial volume at the mandible angle area changed smaller, this changes make the face slimmer.

In chin down position photo the change made by the cheek bone reduction can be well illustrated. In the after surgery photo the zygoma and the zygomatic arch are smaller compared to the before surgery photo, the width of the middle face is narrowed, the facial width lateral to the brows is reduced and the facial outline at both side are straighter. The tailored differentiated zygomatic arch reduction granted the shape and size of the zygomatic arch more symmetric between the two sides.

“Big face” can be reduced to a “small face” by surgical procedures. A surgical plan should be carefully made for face reduction according to the individual situation. Pick the girl in this page as an example, her whole face was wide especially at the middle of the face. A simple mandible reduction and masseter muscle resectomy are not suitable for her situation, because the narrow down of the lower face will make the middle face looks ever wider, destroying the balance of the facial structure. In order to make the face like this girl smaller lower face reduction surgeries must be combined with the cheek bone reduction surgery.

Beside the size another indication leading this girl to do face reduction is that her facial organs were too crowded. This phenomenon is well depicted in this girl’s before surgery photo. As her facial size was big her facial organs seemed crowded in the middle of her face. After surgery the size of her face is smaller. Though the distance in between her facial organs remains the same her facial organs are scattered out visually. This change also improves this girl’s feature very much. So it is very important to control the indications and to make a careful surgical planning in the surgical reduction of “big face”.
