There are some surgery procedures that can reduce the zygomatic arch and zygoma. Some surgeons still utilize Frontal End Zygoma and Zygomatic Arch Osteotomy or Frontal and Rear End Zygoma and Zygomaitc Arch Osteotomy. As these two surgical procedures can not make the arch of the zygoma flat therefore no natural shape of the zygomatic arch can be achieved after surgery. Also the side effect of post surgery facial sagginess caused by the Frontal End Zygoma and Zygaomtic Arch Osteotomy scared away the potential candidates from going into the operation room. Dr. Fushun Ma’s method of Combined Osteotomy Zygoma and Zygomatic Arch Reduction which was put forward during his PhD Degree studies and his later on version of Zygoma and Zygomatic Arch Smooth  Reduction avoid the side effect of facial sagginess. The facial contour after reduction surgery using these two method looks more natural.

The beautiful girl in this page underwent Zygoma and Zygomatic Arch Smooth Reduction Surgery. This girl was originally good looking only the zygomatic bone and zygomatic arch are overgrowth giving a bony facial structure. After Zygoma and Zygomatic Arch Smooth Reduction Surgery the middle third of the face is narrower and her face looks soft and smoother.

After reduction the zygomatic process is no longer protruding to the front-lateral. The size of the zygomatic bone is smaller. The facial indentation below the zygomatic arch disappeared.

Background Story

This beautiful girl has a pair of big smart eyes. At the first glance people will notice her beautiful eyes instantly. She doesn’t speak a lot but her eye can tell you what she want to say. The first time she saw Dr. Ma for consultation and the second time for doing the surgery. She didn’t demand for many goals but make her face smaller and natural because she didn’t like her wide face. She let Dr. Ma help her planning her surgeries. Dr. Ma realized that she know what she wanted. However Dr. Ma also sensed the burden of responsibility when she let Dr. Ma make decisions for her. It is a great relieve when hearing a positive feedback from her regarding her post surgery outcome.

When Dr. Ma checked her CT and her facial structure he realized that her face was generally not big while her zygomatic bone and zygomatic arch are bulging out forming a facial indentation both below and above the zygomatic arch. The surgery must reduce the size of the zygomatic bone and the zygomatic arch and diminish the facial indentations. So the Zygoma and Zygomatic Arch Smooth Reduction surgery was selected for her. Her surgery went through very well and so is her post operation recovery. The post surgery photos displayed in this page was toke 1 year after her operation.

Photo analysis of Zygoma and Zygomatic Arch Smooth Reduction Surgery

From the pre surgery photo in the front view the mid face is wide with a bulging out zygoma and zygomtic arch causing facial indentations both above and blow the zygomatic arch. The whole face is in a diamond shape. Post operatively the side facial outline is smooth, no obvious bulging out or indentation. After surgery the temporal and cheek region is fuller. This fuller effect in this areas is not by any means of augmentation but by the reduced size of zygomatic bone and zygomatic arch. And this reduction made the lateral facial surface even. On the contrary if a temporal and cheek augmentation is employed to make the facial outline smoother then the whole face will be wider or even end up in a swelling look after surgery.

In the right and left 45 degree angled before surgery photos the under zygomatic arch indentation is obvious as shown by the white arrow. The area of the zygomatic process is big as shown by the red arrow on the left side of the face and the zygomatic curvature protrudes to the lateral as shown by the red arrow on the right side of the face. After surgery the zygoamtic process becomes smaller and the zygomatic arch is no longer protruding to the lateral. The overall look of the face is more round and less bony.

In the chin up position pre operative photo the area between the lateral canthouses and the ears is fuller and bulging out to the lateral compared to the post surgery photo. This is not because the soft tissue shrinkage in these areas but the structural change of the zygomatic bone and the zygomatic arch.

No matter in which angle of the face as showing in this page’s photos there is no clue of facial sagginess happened comparing them pre and post surgically. On the contrary if we exam the photos carefully we could tell that when the facial bone is reduced after surgery the facial soft tissue is tighter and more lifted giving a younger looking. This kind of change can also be noticed in other facial bone reduction comparison photos in this website indicating that a right facial bone reduction surgery do lift the face somehow and make the surgery receiver younger instead of causing any facial sagginess. Another most frequently asked quest in everyday consultation is that if the facial bone reduction surgery accelerating aging. In Dr. Ma’s opinion this is just a rumor from guessing. We all know that in China the facial bone reduction surgery gets popular only less then 20 years ago. So almost all these surgery receivers are under the age of 50, giving the fact that most facial bone reduction surgery receivers do their surgery at the age under 30 years old, they are still not old yet. Then we can’t have enough data to support the hypothesis of facial bon surgery accelerating aging. However Dr. Ma encountered many patients who had facial bone injury of facial bone surgery for some other reason on one side of the face, many years later or when they get old the injured side or operated side of the face looks younger and less saggy compared to the other side. This could be due to the scar contraction of the facial soft tissue caused by the injury. And this contraction theoretically could prevent the facial sagginess and aging process.
